3) Fill out form

4) Sign form

5) Turn form into the HBC church office.

Note: If you are having trouble with downloading or printing a form hard copies are available in the church office.

HBC Sports Ministry Form
	Form used for signing up for HBC sports ministry leagues and other church activities.

Facilities Scheduling Form
	Form needed to use church facilities like the gym, fields, and kitchen.

Parent and Guardian Consent Form - Single Activity
	Consent form needed for minors attending HBC Church Sunday school functions, Awana functions, or other Church sponsored functions, activitys, sports, or outings on or off HBC Church properties.

Parent and Guardian Consent Form - Annual
	Consent form needed for minors attending HBC Church Sunday school functions, Awana functions, or other Church sponsored functions, activitys, sports, or outings on or off HBC Church properties.

Web Consent Form
	Consent form needed to put personal information or a photos of someone on the HBC website.

Photograph Consent Form
	Consent form needed to display a photo on HBC ministry documents and materials.

Sponser Release Form
	Form needed for church sponsers, teachers, leaders, and instructors for church events.
